Out-of-network information

  1. Contact your insurance company to find out what your out-of-network benefits are.
    1. Call the number on the back of your insurance card.
    2. Ask them to confirm your out-of-network mental health benefits for individual therapy. 
    3. Tell them the billing code for a 53-minute session (90837).
    4. Tell them the fee you will be charged ($250) each session. 
    5. Ask what you will be reimbursed for each session.
    6. Ask what you need to give them to get this reimbursement.
    7. Ask where you need to send your claims
    8. Ask how long reimbursement will take
    9. Document their answer, and name or id number, in case you have any problems later.
  2. Ask your therapist for a “super bill”, which is a receipt for your services. This will contain the following information:
    1. Your name
    2. Your provider’s name
    3. Dates of service
    4. Cost of service
    5. Diagnostic codes (mental disorder diagnoses)
    6. Billing code (usually 90837)
    7. Send this information and any other information you insurance company has requested to the address or fax number they gave you.
    8. Make sure to follow up with them promptly if you haven’t been reimbursed in the time period they gave you.
    9. There is often a “timely filing” rule that means that if they don’t get your information within a certain period of time, you won’t be reimbursed.
