Out-of-network information
- Contact your insurance company to find out what your out-of-network benefits are.
- Call the number on the back of your insurance card.
- Ask them to confirm your out-of-network mental health benefits for individual therapy.
- Tell them the billing code for a 53-minute session (90837).
- Tell them the fee you will be charged ($250) each session.
- Ask what you will be reimbursed for each session.
- Ask what you need to give them to get this reimbursement.
- Ask where you need to send your claims
- Ask how long reimbursement will take
- Document their answer, and name or id number, in case you have any problems later.
- Ask your therapist for a “super bill”, which is a receipt for your services. This will contain the following information:
- Your name
- Your provider’s name
- Dates of service
- Cost of service
- Diagnostic codes (mental disorder diagnoses)
- Billing code (usually 90837)
- Send this information and any other information you insurance company has requested to the address or fax number they gave you.
- Make sure to follow up with them promptly if you haven’t been reimbursed in the time period they gave you.
- There is often a “timely filing” rule that means that if they don’t get your information within a certain period of time, you won’t be reimbursed.
